Dry eyes

What happen if I have dry eyes?

The most common eye disorder which can affect an individual eyes is Dry Eyes. Nowadays, dry eyes is being a common eye problem which can affect anyone due to inadequate and unstable tear flow or productions. Understanding tear layers and its mechanism is quite complex as tear structure is made up of various components. The interactions of tear layer which composed of lipid, protein, salts, mucin and water is quite essential to obtain its stability. The outermost lipid layer functions to prevent our tear from evaporating, the middle layer of aqueous supplies moisture to our eyes, and the innermost mucus layer is to allow our tear to stick on the front surface of cornea. When this layer has been interrupted, dry eyes may occurs. There are numerous causes and reason for us to know about unstable tear production and tear inadequacy. Before that, we need to know what are the symptoms that we may experience once we have dry eyes. From there, we can able to detect the causes and risk factors of dry eyes. By determining the symptoms and causes, we are able to manage and treat dry eyes more effectively. So, lets read more to know how can someone know if he or she have dry eyes, what happen if I have dry eyes and what are the reason for you to have dry eyes.

First and foremost, we need to discover how does one feel when they have dry eyes. The basic problem that one may encounter is their eyes feel discomfort after a long day facing at screen, watery eyes, experiencing clear and blur vision on and off. We as an optometrist are professionally well trained and experienced in facing various complaint from our patients about dry eyes symptoms. Despite of that, the prevalence of dry eyes has increased during this pandemic as many of us are assigned to do work from home, which makes us to face screen time more and causes dry eye more in the past year. Remember, blinking helps to moisturize our eyes as new layer of tear across the cornea is distributed when we blink. But when we are facing screen time or doing any near visual tasks for longer hours, our blinking rate will reduce half from the average rate (12-15 blink per minute). We should also understand that dry eye may seem to be common but if it left untreated it can lead to chronic condition affecting the front surface of the eyes known as cornea. Once the eye notices a lack of lubrication and moisture in its surface, this can lead to dry eyes. This can help us to know what happen if we have dry eyes. If we notice any of the listed symptoms as below, thus we should start to take an action to treat it.

Symptoms of dry eyes

One may notice a constant uncomfortable feel in their eyes, following with the dry eyes symptoms such as irritation, burning or scratching sensation, tearing eyes, redness in the eyes, mucus discharge, blurry vision, easily can have eye fatigue and itchiness. We may experience one symptom or few combined symptoms at a time. Thus, understanding the tear layer and its anatomy may help us to know the reason of why we need to keep our eyes lubricated all the time and protect our eyes from foreign particles.

Evaporation and aqueous deficient dry eyes
Types of dry eyes : Evaporation and aqueous deficient dry eyes

The two main type of dry eyes is known as evaporation dry eyes and aqueous deficient dry eyes. The evaporation dry eyes carries 85% of the reason for dry eyes to happen. When the lipid production from meibomian gland is insufficient, it will lead our tear to evaporate. Also if there is any blockage or dysfunction of meibomian gland, it will cause an inadequate lipid production, thus, evaporation may occur too. Next, the least common type of dry eye is known as aqueous deficient dry eye, which causes by the insufficient tear production from lacrimal gland and it can be further classified into Sjogren’s and non-Sjogren’s syndrome.

Before confirming on the management and treatment plan for dry eyes, we as an optometrist do also make sure by determining what are the causes of these dry eyes symptoms may occur. It is also important to understand who is at the risk of getting dry eyes easily and it would be helpful for us to monitor them frequently. Dry eyes may develop due to several reasons. It can occur due to aging factor, inappropriate use of contact lenses, side effect of certain medication such as (antihistamines, antidepressants), lack of vitamin A, laser or refractive surgery, hormonal imbalance, eyelid problems, allergic eyes, frequent computer users, and improper blinking effect. Whenever you blink, a new layer of tear film will spread across your cornea. Also severe dry eyes may lead to blurry vision and it can impact your daily life activities.

Dry eyes causes
Causes of dry eyes

To avoid from getting dry eyes, you need to find out how does the dry eyes causes. You can apply some of these simple tips in your daily life. First, do not allow direct air blow into your eyes. Positioning air-condition or fans away from us will help our tears to not evaporate. Usage of artificial tear drops helps to moisturize our eyes and produce a smooth and soothing effect. There are tons of artificial tear drops available in market, and preservative-free tear drop is suggested to use so that it can avoid our eyes from irritation. For more severe dry eyes, you may need to use lubricants or ointment as the thick viscosity in the ointment will stay longer in the eye surface. Studies proved that supplement such as omega-3 and antioxidants helps to treat dry eyes by increasing the production of tears and improve the tear stability. If you have any of the symptoms as listed above, or would like to understand more about dry eyes, please do book an appointment with us today, and get your eyes tested with our professional optometrist in town.

Contact lens solution top 5

We are going to introduce top 5 contact lens solution recommended by our optometrist. It is important to take good care of your contact lenses in order to extend the life of your contact lenses, and protect the safety and health of your eyes. Lens care varies depending on material and wearing schedule. Daily disposable lenses are discarded after a single use and thus require no cleaning. Other lenses require regular cleaning and disinfecting to prevent surface coating and infections.

There are many ways to clean and care for contact lenses, typically called care systems or lens solutions. Two types of solution that are commonly used included:

  • Multipurpose solutions

Multipurpose solutions are the most common method for rinsing, disinfecting, cleaning, and storing soft lenses. Clean your lenses, then rinse and disinfect, all with the same solution. When you are ready to wear the lenses, rinse them again.

  • Hydrogen Peroxide solutions

Hydrogen peroxide can be used to disinfect contact lenses. Care should be taken not to get hydrogen peroxide in the eye because it is very painful and irritating. With “two-step” products, the hydrogen peroxide must be rinsed away with saline before the lenses may be worn. “One-step” systems allow the hydrogen peroxide to react completely, becoming pure water. Thus “one-step” hydogen peroxide systems do not require the lenses to be rinsed prior to insertion, provided the solution has been given enough time to react.

Being concerned with your eye health and contact lens care, Avizor, the expert in solutions, has a single solution for every problem and occasion. Avizor is a laboratory in Madrid, Spain dedicated to the manufacturing of solutions for the cleaning and maintenance of all types of contact lenses. Avizor have been working in this field since 1985. Over the years, Avizor considered to be one of the best contact lens solution brand available.

As for multipurpose solution, there are two different types of solution to suit for different types of contact lens wearer. This include the following:

1) Avizor Unica Sensitive: “In a class of its own”

Avizor Unica Sensitive

For users with sensitive eyes: With the addition of extraordinary  lubricant agent (Sodium Hyaluronate or Hyaluronic acid), the solution has the following effects:

  • Protect cornea thanks to the prolonged time in the eye
  • Improved tear film stability-  Hyaluronic acid is one of the most moisture-absorbent molecules in nature.
  • Increases the hours of comfortable use- Hyaluronic acid easily spread over the eye during blinking.
  • Accelerates healing time preventing irritation and redness caused by the mechanic action of the lens.
  • Clarify your vision by reducing the ocular symptoms of dry eye.

2) Avizor Alvera with Aloe Vera: “Protects in the most natural way”

Top 5 contact lens solution recommended
  • especially designed and tested for use with silicone hydrogel lenses.
  • The use of contact lenses can alter the physiology of the ocular tissue and may create intercellular spaces allowing the entry of foreign substances.
  • Aloe Vera creates a film over the epithelium that prevents the entry of these substances that may damage the epithelium, thus reducing the risk of irritation.
  • removes lipids more effectively, but also reduces the affinity of lipids to adhere to the surface of the contact lenses, which considerably increases hygiene and comfort in the use of silicone hydrogel lenses.

3) On the other hand, Hydrogen Peroxide solution produced by Avizor is called Ever Clean.

Lens Solution

It is a revolutionary “One-step” system for the cleaning and disinfection of all types of contact lenses (Soft contact lenses and Rigid Gas Permeable lenses). Ever Clean contact lens solution is the result of eight years of research into creating a product to clean all contact lenses throughly without the need for rubbing in only 2 hours.

How it works?

Thanks to its 2 layer table EVER CLEAN has double action:

The white layer performs the cleaning action from the first movement, while eliminating lipids and proteins from the surface of the lens.

The green layer acts slowly and extensively controlling the disinfection and neutralization of the hydrogen peroxide.

On the whole, the Ever Clean tablet performs a powerful disinfecting and cleaning action after which the peroxide is transformed into a preservative-free saline solution where the lenses are stored and ready to use.

Ever Clean is specially recommended for users with:

1. Problems of deposit accumulation

  • No more deposit due to a greater capacity of cleaning and disinfecting.

2. Problems with allergies

  • After neutralization the final solution contains no preservatives.

3. Problems manipulating the contact lenses

  •  As you no longer have to rub, users are less likely to damage or tear their lenses.

At Malaya Optical, we as an optometrist have almost encounter dry eye patient everyday. With certain latest techniques and technologies we able to diagnose, assess and treat dry eye in more efficient way. Below are the few dry eye assessments that we usually apply at Malaya Optical.

This process is made easy by taking a history of patients symptoms, signs and complaints related to dry eyes. By accumulating the potential causes and symptoms of dry eye, eye care practitioner could able to determine the suitable management and treatment for it. Those who are suffering from dry eyes, it is important for you to make an appointment and visit our optometrist.

We will be running out few tests accurately according to your current eye conditions under dry eye assessments. Those who are not aware of having a dry eye syndrome, we also could able to diagnose it start from a comprehensive eye examination. Here, we use few number of strategies and skills to assess the causes and severity of your dry eye condition and from there we will develop a dry eye treatment appropriately.

Coeneal miars in NIBUT test

The very first signs that we able to encounter before even a patient could able to complaint to us about their dry eye is by doing a NIBUT (non-invasive tear break up time) test. The cut off value to distinguish a normal patient and a dry eye patient is 12 seconds in NIBUT test (Vidas Pauk 2019). The easiest way to conduct this test is with auto refractor meter. Once their corneal miars is clearly visible on auto refractor meter, patients can start to blink and hold their blink until they could. Meanwhile, the eye care practitioner requires to count how many seconds does a patient can hold their blink or how many seconds does their corneal miars does not get distorted.

The next clinical examination that an optometrist can do to assess dry eyes is with slit lamp examination. This exam allows our optometrist to screen our eyes in anterior and posterior level. This test is done at microscopic level, by examining the eye health include the eyelids, tear layer, conjunctiva, pupil, iris, lens, sclera, cornea, retina and also optic nerves. It is easy to assess the redness of patients eyes and its severity by using a slit lamp biomicroscopy.

Any debris or dust may cause irritation while we blink and these small particles will move around our tear layer when we blink. Uneven tear layer may cause dry eyes, and moreover dust and debris which move around our tear layer causes additional discomforts.

Redness in the eye due to dry eye

Evaluation of the tear film and cornea: by putting a drop of sodium fluorescein (a yellow dye) into the eyes which mixes with the tears to give a clearer indication of the quality of tear film, and any signs of corneal damage due to excessive dryness.

Other than that, slit lamp also will help us to measure the tear meniscus which forms as a pool and accumulates at the bulbar conjunctiva in the inferior lid margin. The normal height of tear meniscus is approximately 0.3 to 0.4mm. A measurement scale is done with an adjustable slit beam height.
